📋 Description

• Make home visits to clients in designated territories.
• Diagnose and measure mobility, ROM, gait, strength, balance.
• Provide wound care with evidence-based dressings.
• Develop care plans with manual therapy and gait training.
• Continuously assess and revise PT care with nursing in team.
• Educate clients and families to promote health and independence.
• Document observations in EMR on a tablet.

🎯 Requirements

• Current STATE Physical Therapist (PT) license.
• At least one year of PT experience.
• Graduate from APTA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
