📋 Description

• Make home visits to clients in designated geographic territories.
• Conduct mobility, ROM, gait, strength, balance assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Create care plans with manual therapy, gait training, and balance.
• Continuously assess and revise the PT care plan with nursing.

🎯 Requirements

• Current New Jersey Physical Therapist license.
• Minimum of 1 year PT experience.
• Graduation from an APTA-accredited program.
• Ability to work independently, manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
