- Conduct PT assessments in facility and home visits to define care needs.
- Actively participate in IDT meetings to develop participant care plans
- Deliver and document interventions per care plans
- Provide active/passive exercise, gait training, ADLs, and related treatments
- Supervise Therapy Assistants, providing training and guidance
- Train staff, participants, and caregivers on therapeutic equipment use
π― Requirements
- Bachelor's degree in physical therapy; master's/doctorate preferred
- Two years in a clinical setting with frail/elderly population preferred
- Valid California PT license or license eligibility
- CPR and First Aid certification within 180 days of hire
- Maintain CPR/First Aid certification thereafter (preferred)
- Outpatient/acute/inpatient/ skilled nursing experience desired
