  • Conduct PT assessments in facility and home visits to define care needs.
  • Actively participate in IDT meetings to develop participant care plans
  • Deliver and document interventions per care plans
  • Provide active/passive exercise, gait training, ADLs, and related treatments
  • Supervise Therapy Assistants, providing training and guidance
  • Train staff, participants, and caregivers on therapeutic equipment use

🎯 Requirements

  • Bachelor's degree in physical therapy; master's/doctorate preferred
  • Two years in a clinical setting with frail/elderly population preferred
  • Valid California PT license or license eligibility
  • CPR and First Aid certification within 180 days of hire
  • Maintain CPR/First Aid certification thereafter (preferred)
  • Outpatient/acute/inpatient/ skilled nursing experience desired
