Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, transfers, balance, strength, endurance.
• Implement wound care with evidence-based dressings and topical agents.
• Develop care plans with manual therapy, gait training, balance.
• Assess and revise PT care plan; coordinate with nursing.
• Educate clients and families to promote health, safety, independence.
• Document observations and interventions using a touchpad tablet.

🎯 Requirements

• NJ Physical Therapist license (PT).
• Minimum of one year PT experience.
• Graduate from an approved PT program (APTA/CAHEA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
