📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, strength, balance, safety.
• Implement wound care with evidence-based dressings.
• Develop and revise individualized care plans, including manual therapy, gait training, balance.
• Educate clients and families to promote health, safety, independence.
• Document observations and interventions using a tablet EMR.

🎯 Requirements

• AZ Physical Therapist license (active).
• Minimum one year of PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
