Make home visits to clients in designated geographic territories.
Perform mobility, gait, balance, strength, and safety assessments.
Implement wound care and dressings using evidence-based methods.
Develop and implement individualized care plans with manual therapy and gait training.
Continually assess and revise the PT care plan with nursing in a multidisciplinary team.
Educate clients and families in rehabilitative care for health and independence.

🎯 Requirements

VA Physical Therapist license (PT)
Minimum 1 year PT experience
Degree from APTA/CAAHEA-accredited program
Ability to work independently and manage time
Strong interpersonal and organizational skills
Proficient with EMR systems
