- Conduct PT assessments at the facility; occasional home visits.
- Actively participate in IDT meetings to develop participant care plans.
- Deliver and document interventions per care plans.
- Provide active and passive exercise, gait, and ADL training.
- Supervise Therapy Assistants and train staff on FMP.
- Provide training to participants and caregivers on equipment.
🎯 Requirements
- Bachelor's degree in physical therapy; master's/doctoral preferred.
- Two years in clinical setting with frail/elderly population preferred.
- Valid California Physical Therapy License or license eligibility.
- CPR and First Aid certification within 180 days of hire; certification on hire preferred.
- Outpatient/acute/inpatient/SNF experience preferred.
