Physical Therapist

📋 Description

• Conduct PT assessments in facility and home visits to define care needs and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per participant care plans.
• Provide active and passive exercises, gait training, and ADLs.
• Supervise Therapy Assistants and guide Functional Maintenance Programs (FMP).
• Train staff, participants, and caregivers on therapeutic equipment.

🎯 Requirements

• Bachelor's degree in physical therapy; master’s/doctoral preferred.
• Two years in clinical setting with frail or elderly population preferred.
• Valid CA Physical Therapy License, or eligibility.
• CPR and First Aid certification within 180 days of hire; preferred at hire.
• Outpatient/acute/inpatient or SNF experience preferred.
