📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, ROM, transfer status, stability, balance, and safety assessments.
• Implement wound care with evidence-based dressings and topical agents.
• Develop care plans including manual therapy, gait, and balance training.
• Continuously assess and revise the PT care plan with nursing and multidisciplinary team.
• Accurately document observations and interventions using a touchpad tablet.

🎯 Requirements

• Current AZ Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from APTA-approved program or AMA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
