📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, balance, and safety assessments.
• Implement wound care with evidence-based dressings and protocols.
• Develop individualized care plans, including gait and balance training.
• Continually assess and revise PT care plans with nursing in the team.
• Educate clients and families on rehabilitative care and safety.

🎯 Requirements

• Pennsylvania PT license.
• One year of PT experience.
• Graduate from an APTA-approved or AMA/CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• EMR experience preferred.
