Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, transfer status, balance, safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement care plans with manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with the nursing team.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• Active, current license to practice Physical Therapist in PA.
• Willingness to travel to cases as assigned.
• Excellent assessment, documentation, and written/verbal communication.
• Calm, reassuring presence with active listening skills.
• Home health experience welcomed; training provided.
