Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests for mobility, ROM, balance, strength, and safety.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and update individualized care plans (manual therapy, gait training, balance).
• Collaborate with nursing in the multidisciplinary care plan.
• Educate clients and families on rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• Current New Jersey Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from an APTA-approved program or CAAHEA accreditation (with transcript).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
