Physical Therapist

📋 Description

• PT assessments in facility and home to define care needs and goals
• Join IDT meetings to develop participant care plans
• Deliver and document interventions per care plans
• Provide active and passive exercise, gait training, ADLs, and related treatments
• Supervise Therapy Assistants and guide FMP implementation
• Train staff, participants, and caregivers on therapy equipment use

🎯 Requirements

• Bachelor’s degree in physical therapy; master’s/doctoral preferred
• Two years clinical experience with frail/elderly population preferred
• CA Physical Therapy license or eligibility
• CPR and First Aid certification within 180 days of hire
• Maintains CPR/First Aid certification thereafter; hire preferred
