Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, balance, strength, gait, and safety assessments.
• Implement wound prevention and care using evidence-based dressings.
• Develop individualized care plans with manual therapy and gait training.
• Assess and revise PT care plans; coordinate with nursing on the care plan.
• Educate clients and families on rehabilitative care to promote safety and independence.
• Document observations and interventions on a tablet.

🎯 Requirements

• A current NJ Physical Therapist (PT) license.
• At least one year of recent PT experience.
• Graduation from APTA-approved program per transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
