Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Implement wound care and protection with evidence-based dressings.
• Develop and implement individualized care plans (manual therapy, gait, balance).
• Assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• A current NJ Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduate of an APTA or AMA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills with EMR experience preferred.
