Physical Therapist

📋 Description

• Conduct PT assessments in facility and home to define goals.
• Participate in IDT meetings to plan participant care.
• Deliver and document interventions per care plans.
• Provide active/passive exercise, gait training, ADLs.
• Supervise Therapy Assistants and implement FMP.
• Train staff and caregivers on use of therapeutic equipment.

🎯 Requirements

• Bachelor’s in Physical Therapy; master’s/doctoral preferred.
• 2 years clinical experience with frail/elderly population preferred.
• Valid California PT license or license eligibility.
• CPR and First Aid certification within 180 days of hire.
• Outpatient/acute/inpatient or SNF experience preferred.
