Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, joint range, balance, gait, strength).
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait and balance training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• Active and current PA Physical Therapist license.
• Previous home health experience welcomed, but not required. We will train you.
• Ability to travel to cases as assigned.
• Calm, reassuring presence with active listening and strong assessment skills.
• Excellent written and verbal communication.
• Graduate of an accredited Physical Therapy program.
