Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, strength, balance.
• Implement wound care and dressings to prevent/repair wounds.
• Develop care plans with exercises, gait and balance interventions.
• Assess and revise PT care plan; collaborate with nursing.
• Educate clients and families on rehabilitative care for safety and independence.
• Document observations and interventions in EMR on tablet.

🎯 Requirements

• Current VA Physical Therapist (PT) license.
• Minimum one year of PT experience.
• Graduation from an APTA- or AMA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
