- Assess PT in facility and home to define care goals
- Participate in IDT meetings to plan participant care
- Deliver and document interventions per plans
- Provide active/passive exercises, gait training, ADLs
- Supervise Therapy Assistants and guide Functional Maintenance Programs
- Train staff and caregivers on therapeutic equipment use
🎯 Requirements
- Bachelor’s degree in physical therapy; master/doctorate preferred
- Two years clinical experience with frail/elderly population preferred
- Valid California PT license or license eligibility
- Must obtain CPR and First Aid within 180 days of hire
- CPR and First Aid certification upon hire preferred
