Continuously assess and revise the plan of care with the care team.
Educate clients and families on therapy to promote independence.
Document observations and interventions using a touchpad tablet.

🎯 Requirements

Licensed Physical Therapist in Massachusetts
Minimum 1 year of clinical PT experience
Geriatric-focused, patient-centered care
Experience in outpatient or home health settings
Strong documentation using digital tablets
