📋 Description

• Make home visits to clients in designated territory.
• Perform mobility, ROM, transfer, balance and safety assessments.
• Implement wound care and protective interventions as needed.
• Develop and revise individualized care plans, including gait/balance training.
• Educate clients/families on rehab and home safety.
• Accurately document observations and interventions using an EMR/tablet.

🎯 Requirements

• VA Physical Therapist license.
• Graduate from an APTA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Proficient with electronic medical records (EMR).
