- Conduct PT assessments in facility and home to define needs and goals
- Participate in IDT meetings to develop care plans
- Deliver and document interventions as per care plans
- Provide active/passive exercise, gait training, ADLs, modalities
- Supervise Therapy Assistants and support FMP implementation
- Build long-term relationships with participants for ongoing care
🎯 Requirements
- BS in physical therapy; masters/doctorate preferred
- 2 years in clinical setting with frail/elderly preferred
- Valid California PT license or eligibility
- CPR and First Aid cert within 180 days of hire
- Outpatient/acute/inpatient or SNF experience preferred
