Pharmacy Liaison

📋 Description

• Provide full medication-management services to hospital patients.
• Work with hospital clinicians to serve patients face-to-face and by phone.
• Educate patients on how hospital services improve their care.
• Support outpatient retail and specialty prescription needs.
• Grow the specialty pharmacy patient population.
• Ensure patients stay on track with regimens; manage refills.

🎯 Requirements

• State-registered pharmacy technician; CPhT preferred but not required.
• Required 2 years of experience; 3-5 years preferred.
• High School Diploma or GED required.
• Strong interpersonal skills; proficient with Microsoft Excel and Word.
• Knowledge of storage/handling for hazardous or refrigerated meds.
• Ability to work independently and as part of a clinic team.
