Pharmacy Liaison

📋 Description

• Provide comprehensive medication management to hospital patients.
• Collaborate with clinicians to serve patients face-to-face and by phone.
• Educate patients on hospital services and care improvements.
• Support outpatient retail and specialty medications.
• Ensure adherence and manage refill requests.
• Communicate with the centralized Patient Service Center and records.

🎯 Requirements

• State-registered pharmacy technician; CPhT preferred but not required.
• 2 years of pharmacy technician experience; 3-5 preferred.
• High School Diploma or GED required.
• Strong communication, organization; proficient in Excel and Word.
• Willing to obtain required licensure within timeframe.
• Knowledge of transplant/hepatitis C/infectious disease/oncology a plus.
