Pharmacy Care Coordinator

📋 Description

• Outbound calls to introduce services and engage patients.
• Schedule pharmacist consultations and coordinate follow-ups.
• Liaise with patients, pharmacies, and providers to resolve barriers.
• Refer to enhanced service pharmacies and support onboarding.
• Assist enrollment in medication assistance programs.
• Document patient interactions and care coordination in systems.
• Collaborate with Care Management Pharmacists on adherence initiatives.
• Stay familiar with CKD meds and insurance concepts.
• Uphold HIPAA and privacy standards.
• Other duties as assigned.
• Success measured by patient engagement and improved adherence.

🎯 Requirements

• High school diploma or equivalent.
• Strong interpersonal and phone communication skills.
• Healthcare, pharmacy, care coordination, or patient support experience.
• Strong organizational skills and attention to detail.
• MS Office Suite including Outlook and Teams.
• Must have reliable home internet with at least 25 Mbps down and 10 Mbps up.
• Evergreen provides remote telephony tools and equipment.
• Dedicated space to protect PHI/HIPAA information.
• Certified pharmacy technician (CPhT) or similar credential.
• Experience in telephonic or call-center environment.
• Familiarity with chronic disease meds and coverage terminology.
