- Attend to daily living and safety needs of participants in PACE center and home.
- Contribute to a center experience that participants want to spend time in; foster belonging.
- Support participants' physical, social, and recreational needs.
- Follow established SOPs; support care outcomes and documentation.
- Record activity and participant care information accurately on forms.
- Provide safety guidance and care plan instruction to participants.
π― Requirements
- Completion of accredited vocational nursing assistant course or comparable care experience.
- High School diploma/GED.
- Knowledge of PACE program.
- Ability to work without close supervision and exercise independent judgment.
- Knowledge of physical, mental, and social needs of frail older adults.
- Minimum of one year experience working with the frail or elderly.
