Per Diem Registered Nurse - Hospice

📋 Description

• Follow designated care plan per patient needs.
• Make home visits to clients in Chester, Delaware, and Philadelphia counties, PA.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to the Clinical or Client Services Manager.
• Document doctors' orders and perform client assessments.
• Utilize a touchpad tablet to document observations, interventions, and evaluations.

🎯 Requirements

• Current PA Registered Nurse license.
• Minimum one year of recent clinical (medical/surgical) experience.
• Prior home care experience preferred, but not required.
• Graduation from an accredited nursing program (transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; solid computer skills; EMR experience preferred.
