Per Diem Home Health Medical Social Worker

📋 Description

• Make home visits to clients in designated territories.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for client and family needs.
• Advocate for clients and families to navigate the community system.
• Develop and maintain the total plan of care with the multidisciplinary team.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Master’s Degree in Social Work.
• Licensed Social Worker (LCSW or LICSW) in Massachusetts.
• Prior home care experience preferred, but not required.
