Make home visits to clients in designated geographic territories.
Provide clinical social evaluations and plan interventions based on evaluation findings.
Identify community resource referrals to address client and family needs.
Advocate for both the client and family to navigate the community system.
Participate in the total care plan with BAYADA's multidisciplinary team.
Document observations and interventions using a tablet.

🎯 Requirements

A Master’s Degree in Social Work.
Licensed Social Worker (LCSW or LICSW) in Massachusetts.
Prior home care experience is preferred, but not required.
Integral member of a multi-disciplinary health care team.
