Per Diem Field Registered Nurse (RN) (Saratoga County, NY / Field-based)

📋 Description

• Conduct initial client assessments to determine care needs.
• Train and supervise non-skilled caregivers in personal care and daily living tasks.
• Monitor client well-being and report changes to providers.
• Educate clients and families on care strategies and daily living skills.
• Ensure compliance with agency policies and state regulations.
• Maintain accurate, confidential client care documentation.
• Liaise between clients, families, and caregivers to improve communication.
• Participate in ongoing training to stay current with home care best practices.

🎯 Requirements

• BSN with NYS license or ADN with homecare experience.
• 1-2 years nursing experience in homecare.
• Geriatric or disability care experience.
• Travel within Albany and surrounding areas as needed.
• Knowledge of state and federal home health regulations.
• Strong interpersonal and communication skills.
• Proficient in MS Office; EHR experience and UAS familiarity.
• Valid driver's license and reliable transportation.
