Monitor clients' wellbeing and report changes to healthcare professionals.
Educate clients and families on effective care strategies and safety.
Ensure compliance with agency policies and state regulations.

🎯 Requirements

BSN with NYS license or RN with homecare exp.
1-2 years nursing experience in homecare.
Geriatric care or disability experience.
Able to travel within Albany, NY and surrounding areas.
Knowledge of state and federal home health regulations.
Proficient in EHR systems.
Proficient in MS Office Suite (Word, Excel, Outlook).
Strongly preferred bilingual in Spanish, Russian, Chinese, Korean, or Bengali.
