Patient Navigator

📋 Description

• Act on referrals from office visits and schedule entries; prioritize BCBS/UHC.
• Document referrals in the EMR from initiation to closure.
• Collect documentation from patient specialist visits.
• Identify SDOH needs and hand off to care team.
• Schedule appointments with internal and outside specialists.
• Facilitate referrals between care team members.

🎯 Requirements

• 2–3 years in medical field; MA or Quality Assistant background preferred.
• Experience in patient-facing roles, especially telephonic outreach.
• Experience in primary care practices preferred.
• Experience with EHRs: Epic, eClinicalWorks, Athena.
• Proficient with Microsoft Office Suite.
• Bachelor’s degree in healthcare field preferred.
