- Visit clients at home to obtain feedback; ensure satisfaction and follow care plan
- Report deviations or care plan requests to the clinical or client service manager

- Demonstrated strong interpersonal skills
- Demonstrated track record of taking on more responsibility with positive results and growth
- Prior health care or home/social service experience preferred
- Competence in basic PC skills
- Must have reliable transportation
