Patient Advocate

📋 Description

• Serve as the primary, ongoing point of contact for Medicare patients.
• Build longitudinal relationships with empathy, respect, and follow-through.
• Help patients understand care plans, appointments, benefits, and resources.
• Conduct person-centered assessments of life context, goals, barriers, and needs.
• Perform SDOH risk assessments and identify unmet social needs.
• Develop individualized action plans aligned to initiating-visit problem or high-risk condition.

🎯 Requirements

• 3+ years in patient advocacy, care coordination, case management, or healthcare navigation.
• Strong understanding of U.S. healthcare system, especially Medicare.
• Experience with older adults or medically complex populations.
• Excellent verbal, written, and interpersonal communication skills.
• Comfortable using EHRs, CRMs, and patient messaging tools.
• Ability to work independently in a remote environment.
