Primary ongoing contact for Medicare patients
Build empathetic, long-term patient relationships
Help patients understand care plans, appointments, benefits, resources
Assess life context and barriers with a person-centered approach
Develop action plans aligned to initiating-visit problems or high-risk conditions
Coordinate care across providers and track outcomes

🎯 Requirements

3+ years in patient advocacy, care coordination, or healthcare navigation
Strong understanding of US healthcare system, esp Medicare
Experience with older adults or medically complex populations
Excellent verbal, written, and interpersonal communication
Comfortable using EHRs, CRMs, and patient messaging tools
Ability to work independently in a remote environment
