- Serve as the main contact for new participants during the first 90 days and beyond.
- Facilitate care coordination from pre-enrollment to enrollment with Enrollment and IDT teams.
- Review participant tracking tools to identify needs and coordinate with IDT for timely action.
- Oversee monthly new enrollee orientation with WelbeLink, vitals, center tour, and IDT intro.
- Conduct checkpoints at 30, 60, and 90 days post enrollment with ongoing follow-ups.
- Partner with Behavioral Health and Social Work teams to check in with high-risk participants.
- Identify participants at high-risk for disenrollment and conduct community and center check-ins.
π― Requirements
- Bachelorβs degree in human services, communications, hospitality, or related field; experience may substitute.
- Minimum of four years in customer service or other customer-facing role.
- Highly motivated, self-directed, able to adapt to fast-changing environment.
- Proficiency in data entry and analysis.
- Proficiency in Microsoft Office programs.
- Ability to manage sensitive conversations with compassion and discretion.
