- Liaise with interdisciplinary team and providers to ensure high-quality care
- Support onboarding and service-recovery projects, incl. satisfaction surveys
- Facilitate communication among providers, families, participants, and caregivers
- Provide customer service calls to ensure satisfaction with appointments
- Support the grievance process with high-touch service until resolution
- Attend required meetings and committees and follow up with participants
π― Requirements
- High school diploma or equivalency; experience may substitute
- Bachelor's in communications or relevant field preferred
- Strong organizational, customer service, and communication skills with seniors and IDT
- Ability to present information to groups and leadership
- Self-directed, adaptable, able to act in emergencies
- CPR and First Aid certification within 180 days of hire (preferred)
