πŸ“‹ Description

  • Liaise with interdisciplinary team and providers to ensure high-quality care
  • Support onboarding and service-recovery projects, incl. satisfaction surveys
  • Facilitate communication among providers, families, participants, and caregivers
  • Provide customer service calls to ensure satisfaction with appointments
  • Support the grievance process with high-touch service until resolution
  • Attend required meetings and committees and follow up with participants

🎯 Requirements

  • High school diploma or equivalency; experience may substitute
  • Bachelor's in communications or relevant field preferred
  • Strong organizational, customer service, and communication skills with seniors and IDT
  • Ability to present information to groups and leadership
  • Self-directed, adaptable, able to act in emergencies
  • CPR and First Aid certification within 180 days of hire (preferred)
