📋 Description

  • Act as liaison with IDT and care providers to ensure high-quality care.
  • Assist management with participant experience projects, incl. satisfaction surveys.
  • Facilitate communication among providers, families, participants, and caregivers.
  • Provide customer service to ensure satisfaction with IDT and appointments.
  • Support grievance process with high-touch service and IDT partnership until resolution.
  • Attend required meetings and committees as directed by supervisor.

🎯 Requirements

  • High school diploma or equivalency; professional experience may substitute.
  • Bachelor’s degree in communications or relevant field preferred.
  • Excellent organizational, customer service, and communication with seniors.
  • Ability to present information to various groups and leadership.
  • Highly motivated, self-directed, adaptable, and able to decide in emergencies.
  • CPR and First Aid within 180 days of hire; certification preferred at hire.
