- Act as liaison with IDT and care providers to ensure high-quality care.
- Assist management with participant experience projects, incl. satisfaction surveys.
- Facilitate communication among providers, families, participants, and caregivers.
- Provide customer service to ensure satisfaction with IDT and appointments.
- Support grievance process with high-touch service and IDT partnership until resolution.
- Attend required meetings and committees as directed by supervisor.
🎯 Requirements
- High school diploma or equivalency; professional experience may substitute.
- Bachelor’s degree in communications or relevant field preferred.
- Excellent organizational, customer service, and communication with seniors.
- Ability to present information to various groups and leadership.
- Highly motivated, self-directed, adaptable, and able to decide in emergencies.
- CPR and First Aid within 180 days of hire; certification preferred at hire.
