📋 Description

  • Act as liaison with IDT, care team, and external providers to ensure high-quality care
  • Assist management with participant experience projects, incl. surveys
  • Facilitate communication among providers, families, participants, and caregivers
  • Provide customer service calls to ensure satisfaction with IDT and appointments
  • Support the grievance process with high-touch service and IDT collaboration
  • Continue check-ins with participants to prevent disenrollment and address concerns

🎯 Requirements

  • High school diploma or equivalent; experience may be substituted
  • Bachelor’s degree in communications or relevant field preferred
  • Excellent organizational, customer service, and communication with seniors, families, and IDT
  • Ability to present information to various groups and leadership
  • Self-directed, able to adapt quickly and make sound decisions
  • CPR and First Aid certification within 180 days of hire; certification preferred
