Goals-of-care conversations with patients and families
Code status discussions, advance care planning, and POLST/MOLST completion
Family meetings with complex dynamics or surrogate decision-making
Coordinate transitions to home hospice, inpatient hospice, or palliative-supported discharge
Prognostic support and education for primary teams

🎯 Requirements

Active SC licensure as an NP (FNP, AGNP, or AGACNP) or PA-C
DEA registration (or eligibility)
Strong communication skills, especially around serious illness and end-of-life conversations
Comfort working autonomously within a collaborative team
