Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, ambulation, strength).
• Develop and implement individualized care plans (manual therapy, gait, balance).
• Assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care for safety and independence.
• Document observations and interventions in the EMR on a tablet.

Qualifications

• A Virginia Physical Therapist license.
• Minimum 1 year of PT experience.
• Graduate from an APTA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and time-management skills.
• Proficient with computers; EMR experience preferred.
