Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform mobility/ROM, strength, balance, and safety assessments.
• Develop and implement care plans with exercises, gait training, and balance work.
• Continuously assess and revise the PT care plan with nursing and the team.
• Educate clients and families to promote health, safety, and independent living.
• Accurately document observations and interventions in EMR.

🎯 Requirements

• Maryland Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
