Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated territories.
• Assess mobility, ROM, strength, balance, and safety.
• Develop and implement individualized care plans.
• Revise care plans with the nursing team as needed.
• Educate clients and families on rehab and home safety.
• Document care using an EMR tablet.

🎯 Requirements

• Maryland PT license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved or CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
