Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests/measurements: mobility, ROM, transfers, balance, strength, safety.
• Develop and implement individualized care plans including manual therapy, gait training, balance.
• Continually assess and revise PT care plan; coordinate with nursing.
• Educate clients/families in rehabilitative care to promote health and independence.
• Document observations, interventions, and evaluations in EMR/tablet.

🎯 Requirements

• Maryland PT license required.
• Minimum 1 year recent PT experience.
• Graduation from an APTA/CAAHEA approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
