Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform mobility, ROM, balance, and safety assessments.
• Develop and implement PT care plans; gait training and balance.
• Continually assess and revise PT care plan; coordinate with nursing in MDT.
• Educate clients and families to promote health, safety, independence.
• Accurately document observations and interventions in EMR on a tablet.

🎯 Requirements

• Maryland Physical Therapist license.
• Minimum of one year PT experience.
• Graduate of an APTA-approved or AMA-accredited PT program; provide transcript.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, problem solving skills.
• Solid computer skills; EMR experience preferred.
