📋 Description

• Visit clients in designated geographic territories for outpatient care.
• Assess mobility, ROM, strength, balance, and safety.
• Develop and adjust PT care plans; use exercises and gait training.
• Collaborate with nursing for multidisciplinary care.
• Document observations and interventions in EMR.
• Educate clients and families to promote independence.

🎯 Requirements

• Maryland Physical Therapist license.
• Minimum one year of PT experience.
• Graduate of an APTA-approved program or CAAHEA/AMA accredited.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid EMR/computer skills; prior EMR experience preferred.
