Outpatient Physical Therapist - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/ROM, transfers, stability, strength, endurance, balance, and safety assessments.
• Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Continuously assess and revise PT care plans, and participate with nursing in the multidisciplinary care plan.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

🎯 Requirements

• Maryland Physical Therapist license.
• At least one year of PT experience.
• Graduation from an APTA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
