Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform PT exams: ROM, gait, balance, strength, endurance, safety.
• Develop and implement individualized care plans with manual therapy, gait and balance training.
• Continually reassess PT plan; collaborate with nursing on care plan.
• Educate clients and families in rehabilitative care for safety and independence.
• Document care using EMR with tablets.

🎯 Requirements

• Current PA PT license.
• Minimum of one year of recent PT experience.
• Graduate from an APTA-approved PT program; show transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
