- Make outpatient visits to clients in designated geographic territories.

- Develop and implement individualized care plans (manual therapy, gait training, balance).
- Assess and revise PT plans; coordinate with nursing in care plans.
- Educate clients and families on rehabilitative care for health and independence.
- Document observations and interventions using EMR.
🎯 Requirements
- A current PA PT license.
- Minimum 1 year of PT experience.
- Graduate of APTA-approved PT program per AMA/CAAHEA.
- Ability to work independently and manage time.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; EMR experience preferred.
