Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfer, gait, balance, safety).
• Develop and implement individualized PT care plans (manual therapy, gait training, balance).
• Continuously assess and revise PT care plans with the multidisciplinary team.
• Educate clients and families to promote health, safety, and independent living.
• Document observations and interventions in EMR.

🎯 Requirements

• A current DE Physical Therapist - PT license.
• A minimum of one year of recent work experience as a PT.
• Graduation from an APTA- or AMA-accredited PT program per transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
